Common Application - Complete First Year Application Packet(PDF)
Includes the Common Application, information forms, academic requirements and instructions.  The Lehigh University Supplement is not included.

Lehigh Health Professions Program Request (PDF)
Required for all students applying to the Accelerated Medical, Accelerated Dental, and Accelerated Optometry programs.

Instructions and Supplement for the 7 year BA/MD program (PDF)
Required for all students applying to the Accelerated Medical program.

Final Report (PDF)
(Students entering in Fall 2009 or Spring 2010 should use this 2009-2010 form)

Home School Supplement to the Secondary School Report (PDF)
This form is recommended for Home School students who do not have an existing transcript.

International Financial Aid Documents (for non-U.S. citizens):

  • International Student Certification of Finances (PDF)
    Required of all non-US Citizens to verify the amount of your financial contribution to the cost of your education.
  • International students applying for need-based financial aid should submit one of the following:

    CSS/Financial Aid Profile
    You may submit this form online. You will be required to pay a registration fee as well as a fee for each college to which your information will be sent. You may pay by credit card or online check. If your family circumstances prevent payment of this fee, you should contact us for additional information.

    International Student Financial Aid Application (ISFAA) (PDF)
    You may download a copy here and submit by paper to the Office of Admissions.
